Specialist Care

Specialist services are required for long-term conditions or conditions which people are born with or have developed over a period of time. Being diagnosed with a condition or prognosis which will progress over a period of time does not mean an end of an individuals life. Most times the individual will need support to live independently to enable them to be active, comfortable and safe. As part of our specialised care services, following a careful assessment of your health and social care needs, we develop a comprehensive person-centred plan which is regularly reviewed to ensure its suitability to your continually changing health and social care needs.

We support people living with a range of conditions such as: Dementia, Acquired Brain Injury, Reduced Mobility, Arthritis Care, Parkinson’s, Alzheimer’s Disease, Palliative Care, Rehab assistance at home, Mental Health, Sensory Impairments and Physical Disabilities.

At CaringHeart Homecare we also work in partnership with other healthcare professionals, including GP, District Nurses and many more in order to understand and meet the needs of our clients with specialist conditions.
